Job Summary

Location: Cape Girardeau MO

Full Time Non-Exempt: 8:00am - 4:30pm Monday - Friday

Minimum Required Qualifications:

Bachelor Degree in a human services - related field or A licensed practical nurse (LPN); or has a least 4 years of experience working in the human services or medical field.
Previous Office Experience
Must be proficient in Microsoft Office
This position requires a valid drivers license proof of continuous auto insurance and must have an acceptable driving record as outlined by agency policy.

About this position:

As a Coordinator for Healthcare Home (HCH) Responsible for coordination of care referral follow up and tracking as well as tracking hospital and residential admissions/discharges in accordance with CCBHO standards.

What your duties would look like as a Support Staff:

Responsible for tracking hospital and residential admissions and discharges for clients not enrolled in healthcare home services.
Complete required follow up after hospital discharge for clients not enrolled in healthcare home services.
Complete psych only discharges and coordinate with medical provider to review and approve discharge documentation.
Assist with external referrals and follow up to ensure referrals are complete.
Document according to clinical policies and procedures DBH CARF ADA and any other federal or state program per agency standards

You know your way around a computer and your advanced computer skills will help you provide our clients and staff with the most current information in turn becoming a key component in providing the best client care. Microsoft Excel skills would be beneficial.
Utilizing your solid Customer Service skills such as compassion empathy positive demeanor effective communication active listening adaptability attentiveness patience self-control conflict resolution and de-escalation skills are necessary and will be always provided to our clients and staff.
