Position summary

UHN is seeking a Manager of Professional Practice (MPP) for the Social Medicine & Community Collaboration Program within the Medical VP Collaborative Academic Practice Portfolio. This role co-designs implements and evaluates models of care that improve access continuity and outcomes for underserved and structurally marginalized populations across hospital primary care and community settings. 

The MPP collaborates with the Family Health Team Mental Health and Addictions Program Emergency Departments and community partners such as IMPACT and Inner-City Health Associates (ICHA) to align care pathways strengthen transitions and reduce system fragmentation. 

The successful candidate will work with interprofessional teams to build shared purpose cohesive practice and psychologically safe learning environments. The MPP supports capacity building quality improvement and change adoption through collaborative problem-solving and collective accountability. 

Reporting to the Director of Professional Practice the MPP advances evidence-informed practice standards and care integration in partnership with staff leaders patients families and community collaborators. The work is grounded in trauma-informed culturally safe and harm-reductionoriented care aligned with UHNs Purpose Values and Principles. 

Duties

  • Lead coordinate and advance collaborative care models for underserved and marginalized populations in partnership with clinical programs primary care emergency outreach and community organizations
  • Establish and sustain interprofessional practice development structures including education coaching mentoring and reflective practice
  • Support and align care pathways and transitions across hospital primary care emergency and community settings to reduce fragmentation and improve continuity
  • Plan and guide quality improvement and evaluation initiatives; translate findings into service planning and practice enhancement
  • Synthesize and apply evidence data and lived/living experience insights to inform decision-making and practice refinement
  • Cultivate and reinforce a respectful learning-focused team culture grounded in trauma-informed culturally safe and harm-reductionaligned approaches

Qualifications :

  • Masters degree in a regulated health profession
  • Current registration in good standing with relevant regulatory college
  • BCLS Healthcare Provider 
  • ASIST Suicide Prevention certification 
  • Overdose response training preferred
  • Mental health/addictionsrelated certification (e.g. CACCF CPMHN(C) Concurrent Disorders ABA PIECES) is an asset 
  • 5 - 8 years clinical experience in Mental Health & Addictions with marginalized/underserved populations 
  • 5 - 8 years clinical experience in medicine (e.g. Internal Medicine Emergency medicine or medical subspecialty) is preferred
  • 3-5 years Advanced Practice Provider Clinical Education or change management experience  is preferred 
  • Experience in a community health or outpost environment is an asset  
  • Expert Understanding of Trauma-informed culturally safe and harm-reductionaligned care approaches
  • Expert understanding of Social determinants of health intersectionality and community-engaged care frameworks
  • Demonstrated understanding of Standards of practice across hospital primary care community and outreach settings
  • Relevant legislation and ethical practice frameworks (RHPA Mental Health Act consent and capacity)
  • Experience with psychotherapeutic and engagement approaches (e.g. CBT DBT ACT MI)
  • Sound understanding of Adult learning principles and methods for capability-building practice development and reflective learning
  • Demonstrated experience with program development and planning to meet regulatory standards
  • Demonstrated experience implementing and sustaining professional practice standards and quality improvement in healthcare environments
  • Strong understanding of integrated care models care transitions and system-level collaboration across sectors
  • Ability to interpret evidence integrate lived/living experience insights and translate data into meaningful practice change
  • Builds shared purpose cohesion and psychological safety across interprofessional teams
  • Skilled facilitator able to navigate competing priorities and support constructive problem-solving
  • Strong planning implementation and change management capability
  • Adaptive and pragmatic in complex and evolving environments
  • Ability to coach and mentor others to support growth and capacity building
  • Effective relationship-builder across hospital primary care community and sector partners
  • Technologically fluent to support documentation education and practice enablement
  • Eligible for academic cross-appointment with U of T and additional academic institutions preferred
