Job Description:

A Brief Overview

Responsible for coordinating and overseeing patient consultations evaluations admissions assessments and care planning for home health and hospice services while ensuring compliance with regulatory standards. It involves educating patients and families collaborating with interdisciplinary teams and promoting services within the organization and community. The position also supports quality improvement initiatives referral management and leadership coverage as needed.

What you will do

  • Conduct thorough evaluations consultations admissions and assessments for new home health and hospice patients.
  • Educate participants and their families on available services program expectations and organizational policies.
  • Maintain accurate up-to-date-documentation in compliance with all state regulations and program requirements.
  • Communicate collaborate and coordinate with families therapists social workers and other members of the interdisciplinary team to ensure high quality care and services.
  • Provide leadership support and coverage as needed.
  • Responsible for promoting and marketing home health and hospice services both within the organization and throughout community to enhance visibility and growth.
  • Facilitate effective communication between referral sources and/or facilities to support a seamless and positive continuum of care
  • Actively participate in the Quality Assurance Performance Improvement (QAPI) process and attend required meetings.
  • Process and evaluate referrals to assess appropriate hospice care settings and level of care to ensure timely and seamless transitions for patients and families.


Qualifications

  • Associates Degree (AA) Graduate of accredited school of nursing Required
  • 1-3 years Acute Care Experience within the Last 5 Years Required
  • 1-3 years Experience with Microsoft Suite (Word Excel PowerPoint Outlook Teams) Required
  • Knowledge of the structure and content of the English language including the meaning and spelling of words rules of composition and a comprehensive working knowledge about the field of home health and hospice including resources available to patients and families. Knowledge of the information and techniques needed to diagnose and treat human injuries diseases and deformities. This includes symptoms treatment alternatives drug properties and interactions and preventive health-care measures. Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment meeting quality standards for services and evaluation of customer satisfaction. Knowledge of human behavior and performance; individual differences in ability personality and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders. Knowledge of principles methods and procedures for diagnosis treatment and rehabilitation of physical and mental dysfunctions. Knowledge of basic math to calculate medication doses. Being aware of others reactions and understanding why they react as they do. Giving full attention to what other people are saying taking time to understand the points being made asking questions as appropriate and not interrupting at inappropriate times. Adjusting actions in relation to others actions. Talking to others to convey information effectively. Using logic and reasoning to identify the strengths and weaknesses of alternative solutions conclusions or approaches to problems. Understanding written sentences and paragraphs in work related documents. Actively looking for ways to help people. The ability to tell when something is wrong or is likely to go wrong. It does not involve solving the problem only recognizing there is a problem. The ability to listen to and understand information and ideas presented through spoken words and sentences. The ability to combine pieces of information to form general rules or conclusions (includes finding a relationship among seemingly unrelated events). The ability to communicate information and ideas in speaking so others will understand. The ability to identify and understand the speech of another person. Knowledge of principles and processes for providing and personal services. This includes customer needs assessment meeting quality standards for service and evaluation of customer satisfaction.
