Our client is a leading PACE (Program of All-inclusive Care for the Elderly) Provider

- Adult day primary care (including doctor and recreational therapy nursing services)
- Dentistry

- Laboratory/x-ray services
- Nursing home care
- Occupational therapy
- Physical therapy

- Social work counseling
Essential job duties include:
- Perform comprehensive in-person assessments leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant
- Coordinate discharge for acute short-term and long-term placement
- No hospital rounds focus on comprehensive care seeing an average of 8 patients a day
- We are seeking Primary Care Physicians in Internal Medicine Family Medicine Emergency Medicine or Geriatrics.
- Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants.
- Board certification preferred.
