CATHOLIC PALLIATIVE CARE SERVICES

Summary & Objective
The Manager Program Palliative Care is responsible for directing and coordinating the overall functions that create a concise clear path for patients to move on the continuum of care from palliative to hospice or other CHS program.
Essential Functions
- Ensure effective communication with Marketing Field Admissions Team Referral Sources and Hospice Interdisciplinary team for assigned facility.
- Serves as the program expert and go to person for hospice physicians hospice clinical staff and assigned hospice admissions staff including marketing team members.
- Coordinates the daily operations between hospice and palliative teams to contribute to smooth admission process.
- Takes proactive steps to minimize delays in delivery of services.
- Regularly communicates with customers/clients to arrange meetings services and discuss any hospice or palliative issues.
- Regularly communicates with management regarding the status to effectively move patients through the continuum of care.
- Identifies staffing needs proactively and communicates with management team.

Other Duties
- Ensures that the program activities are conducted in a manner that is consistent with protocols and are in compliance with Federal State and hospital regulations guidelines and requirements.
- Develops and implements performance improvement activities to respond to service issues and challenges and participates as needed in the Performance Improvement Committee.
- Ability to plan organize and present training activities to diverse employee groups at assigned facility.
- Excellent organizational skills and attention to detail.
- Excellent time management skills with a proven ability to meet deadlines.
- Strong analytical and problem-solving skills.
- Assist with meeting preparation time of day varies. Some may be in the evening.
- Responsible for taking minutes of all meetings conducted at assigned facility.
- Able to establish maintaining and managing effective working relationships and handling delicate political situations.
- Participates in the development and implementation of operating policies and procedures.
- Provides oversees and/or coordinates the provision of training for new and existing staff on applicable operating policies protocols systems and procedures standards and techniques.
- Maintain your required licenses certifications and mandatory skill updates.
- Perform other duties as assigned.
Supervisory Responsibility
- May serve as an interim department leader depending on need
Physical Requirements
- Must be able to lift and/or move up to 50 pounds and push/pull up to 250 pounds walk climb stair or ladders stand on feet for extended periods of time etc.

Requirements
Knowledge & Experience Requirements
- 3-5 years in a medical practice or health system with a profound understanding of hospice and hospital rules and regulations is preferred.
- Clinical discipline RN or APRN preferred.
- Must have knowledge of computer office/clinical software.
- Must be able to read write and understand the English language. Bilingual preferred.
