Registered Nurse/Registered Practical Nurse (Contract)
Position Overview
As a Registered Nurse (RN) or Registered Practical Nurse (RPN) you are passionate about empowering clients and their families to maintain independence through health education and hands-on care. Working collaboratively with the Care Designer and the interdisciplinary care team you will help create a safe innovative and client-centered environment that supports each persons unique goals and needs.
Key Responsibilities
- Conduct comprehensive health assessments for clients and their family members or caregivers.
- Identify care needs and develop personalized goal-oriented care plans.
- Provide health teaching and hands-on training to clients and families on daily living activities such as:
- Personal care
- Medication management
- Mobility and safety
- Support clients and families in navigating the healthcare system and connecting with community resources.
- Monitor and evaluate client progress making adjustments to care plans as needed to ensure best practices and safety.
- Maintain accurate and timely documentation of assessments teaching and care plans.
- Communicate effectively with clients families and the healthcare team to ensure integrated coordinated care.
- Promote safe client- and family-centered care consistent with each individuals home environment and life circumstances.
- Utilize mobile and electronic applications for documentation and communication (e.g. phone email AlayaCare).
Availability
- Status: Casual Contract Position
- Flexibility: Required for scheduling and client visits
Reporting & Relationships
- Reports To: Care Designer
- Collaborates With: Care Designers and interdisciplinary care team
Key Performance Indicators
- Attendance and reliability
- Compliance with AMS (Attendance Monitoring System AlayaCare)
- Quality of client care and documentation
Qualifications & Requirements
- Current registration in good standing with the College of Nurses of Ontario (CNO) as an RN or RPN
- Minimum 35 years of nursing experience in a clinical or community health setting (home care experience is an asset)
- Demonstrated excellence in health teaching and communication with a compassionate client-centered approach
- Two positive professional references
- Valid CPR and First Aid Certification
- Valid drivers license and access to a reliable vehicle
- Ability to travel independently throughout Ontario
- Criminal record check (vulnerable sector clearance)
- Strong written and verbal communication skills in English
- Proficiency with mobile applications and electronic documentation systems
