SUMMARY
The Program Manager will oversee the implementation and management of the Transitions in Care for Justice-Involved Populations grant. This role involves coordinating with various stakeholders managing program activities and ensuring the successful transition of justice-involved individuals into community-based healthcare services. The Program Manager facilitates communication between MedLink providers Medication Assisted Recovery Program Manager Behavioral Health Program Director Court Coordinators carceral staff and others to support patient needs during their transition from incarceration and re-entry into the community.
Key Responsibilities:
- Program Management: Lead the planning implementation and evaluation of the Transitions in Care program.
- Stakeholder Coordination: Collaborate with healthcare providers correctional facilities community organizations and other stakeholders to ensure seamless transitions.
- Grant Compliance: Ensure all program activities comply with grant requirements and reporting standards.
- Data Management: Collect analyze and report program data to measure outcomes and inform continuous improvement.
- Budget Oversight: Manage the program budget ensuring efficient use of resources and adherence to financial guidelines.
- Staff Supervision: Supervise program staff providing guidance support and professional development opportunities.
- Community Outreach: Work alongside the Outreach Team to develop and implement outreach strategies to engage justice-involved populations and promote program services.
- Policy Development: Assist in the development and implementation of policies and procedures related to the program.
Qualifications:
- Education: Associates degree in public health Social Work Healthcare Administration Criminal Justice or a similarly related field related field. Bachelors degree or higher preferred.
- Experience: Minimum of 2 years of experience in program management preferably in healthcare or social services setting. Experience with the justice system including law enforcement court systems or jail systems is highly desirable.
