Job Title: Registered Nurse - Hospice
Work Location: Basking Ridge New Jersey
Rate: $65/hourly/w2
Duration: 3 months (extensions)
Hours Per Week:40.00
Shift: 8:30 am 4:30 pm but flexible with the hours
Duties:
- Performs Patient assessments: Accurately assesses patients appropriateness for hospice care learning needs and progress toward goals.
- Develops the plan of care for the patient and family
- Ensures appropriate utilization of services: Maintains visit productivity standards. Demonstrates knowledge of insurance reimbursement Completes discharge assessments: Performs laptop transfer of data twice daily.
- Completed admission assessment and transfers within 24 hours.
- Completes re-visit documentation and transfers
- Uses age-appropriate teaching methods for patients and families: Evaluates teaching plans and revises them as necessary.
- Documents patient education tools used Assumes responsibility for Case Management: Develops/implements/evaluates plan of care based on evidence-based knowledge.
- Revises care plan as indicated.
- Prioritizes care needs.
- Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines.
- Shows evidence of interdisciplinary communication. Performs HHA supervisions per standards.
- Ensures appropriate utilization of services.
- Follows scheduling process.
- Communicates progress toward goals with IDT.
- Attends and participates in IDT meetings every 2 weeks.
- Evaluates the plan of care and revises as needed Maintains safe environment for patients and co-workers.
- Follows infection control measures and reports infections.
- Demonstrates knowledge of pharmacology and safe medication management. Routinely reconciles medications.
- Maintains current medication orders Attends monthly staff meeting:Participates in PI meetings and activities/committees as requested.
- Accepts other assignments as requested.
- Assumes responsibility for case management: Prioritizes schedule to accommodate patients needs.
- Documents case management activities.
- Demonstrates knowledge of pharmacology and safe medication management.
- Routinely reconciles medications.
- Maintain current medication orders.
- Participates in Performance Improvement Program: Reviews health records and gathers data to support PI activities.
- Attends UR meetings as requested.
- Provides orientation/ mentorship to new employees.
- Cross trains to learn new skills and demonstrates a willingness to teach other Assists with growing agency business: Provides orientation to new employees.
- Accepts other assignments as requested.
- Cross trains to learn new skills.
Skills:
- Must have 2-3 years as RN in home hospice environment
- Must have valid New Jersey drivers license
- BLS - required
- Experience using an EMR
- excellent critical thinking and communication skills

- Bachelors Degree - required
