_{Shift: 8:30 am 4:30 pm but flexible with the hours}

_Duties:

_{Performs Patient assessments: Accurately assesses patients appropriateness for hospice care learning needs and progress toward goals.}
_{Develops the plan of care for the patient and family}
_{Ensures appropriate utilization of services: Maintains visit productivity standards. Demonstrates knowledge of insurance reimbursement Completes discharge assessments: Performs laptop transfer of data twice daily.}
_{Completed admission assessment and transfers within 24 hours.}
_{Completes re-visit documentation and transfers}
_{Uses age-appropriate teaching methods for patients and families: Evaluates teaching plans and revises them as necessary.}
_{Documents patient education tools used Assumes responsibility for Case Management: Develops/implements/evaluates plan of care based on evidence-based knowledge.}
_{Revises care plan as indicated.}
_{Prioritizes care needs.}
_{Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines.}
_{Shows evidence of interdisciplinary communication. Performs HHA supervisions per standards.}
_{Ensures appropriate utilization of services.}
_{Follows scheduling process.}
_{Communicates progress toward goals with IDT.}
_{Attends and participates in IDT meetings every 2 weeks.}
_{Evaluates the plan of care and revises as needed Maintains safe environment for patients and co-workers.}
_{Follows infection control measures and reports infections.}
_{Demonstrates knowledge of pharmacology and safe medication management. Routinely reconciles medications.}
_{Maintains current medication orders Attends monthly staff meeting:Participates in PI meetings and activities/committees as requested.}
_{Accepts other assignments as requested.}
_{Assumes responsibility for case management: Prioritizes schedule to accommodate patients needs.}
_{Documents case management activities.}
_{Demonstrates knowledge of pharmacology and safe medication management.}
_{Routinely reconciles medications.}
_{Maintain current medication orders.}
_{Participates in Performance Improvement Program: Reviews health records and gathers data to support PI activities.}
_{Attends UR meetings as requested.}
_{Provides orientation/ mentorship to new employees.}

_Skills:

_{Must have 2-3 years as RN in home hospice environment}
_{Must have valid New Jersey drivers license}
_{BLS - required}
