_{Key Responsibilities:} _{Patient Coordination & Support}

_{Coordinate care across specialty services to ensure effective and safe patient outcomes.}
_{Assess patients and caregivers self-management skills and support autonomy through education training and referral coordination.}
_{Communicate regularly with patients and caregivers to provide updates and facilitate access to services.}
_{Prioritize referrals and patient outreach based on clinical urgency safety and care quality.}

_{Administrative & Operational Duties}

_{Schedule appointments respond to clinic calls and communicate with patients via phone email and WellApp messaging.}
_{Generate patient letters and complete necessary documentation and forms.}
_{Submit and manage patient travel requests including Letters of Medical Necessity escorts and lodging arrangements.}
_{Verify provider IT access travel and housing needs.}

_{Clinic & Team Support}

_{Collaborate with the Specialty Care Team to develop care plans and optimize healthcare outcomes.}
_{Support front desk operations and ensure clinic coverage for all service lines.}
_{Monitor clinic schedules manage cancellations fill open appointment slots and ensure accurate patient preregistration.}
_{Coordinate educational in-services for clinic staff when needed.}
_{Collect and input coordination data into designated databases and complete reporting as requested by leadership.}

_{Customer Service & Professionalism}

_{Deliver the highest level of customer service and professionalism to patients families and staff.}
_{Advocate for patients at the service-delivery level and respond promptly to their needs.}

_{General Responsibilities}

_{Maintain a clean safe and well-supplied working environment.}
_{Report any missing or defective equipment and potential safety hazards.}
_{Perform additional duties as assigned in a respectful and responsible manner.}

_{Required Qualifications:}

_{Education & Certification:}

_{High school diploma or equivalent Required}
_{Associates degree or 2 years of college in social work healthcare administration or related field
OR 4 years of relevant professional experience may substitute for formal education}
_{Basic Life Support (BLS) certification Required}

_Experience:

_{Minimum of 2 years of experience in a healthcare setting Required}
_{Experience in a primary care or specialty clinic Preferred}
