Job Title: Physical Therapist Assistant (PTA) Home Health
Location: Portland OR 97225 (Coverage Area: West Portland Newberg McMinnville Sherwood Tigard Beaverton and surrounding communities)
Rate: $43 $45/hour (W-2)
Shift: Day Shift Monday Friday 5x8-Hour Days (8:00 AM 4:30 PM)
Employment Type: Travel Contract (13 Weeks)
Providence Health and Services is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) for a 13-week travel contract in Home Health. Youll provide one-on-one in-home therapy under the supervision of a licensed Physical Therapist. This role offers the opportunity to work independently while receiving strong clinical and administrative support.
Key Responsibilities:Deliver in-home therapeutic treatments to patients as directed by a licensed Physical Therapist.
Complete up to six patient visits per day aligned with a 30-point productivity standard.
Document care promptly and accurately using EPIC Remote Client and Rover mobile application.
Travel to patient residences within the assigned territory to provide care.
Collaborate with an interdisciplinary care team to meet patient goals.
Maintain professionalism and empathy in all patient interactions.
Education:
Associates Degree from an accredited Physical Therapist Assistant (PTA) program.
Experience:
Minimum of 1 year of recent home health experience in a skilled setting.
Licensure & Certification:
Active Oregon PTA license (pending approval accepted).
Current Basic Cardiac Life Support (BCLS) certification.
Proficient in mobile charting using EPIC and Rover app.
Able to work independently and manage time efficiently.
Excellent interpersonal communication and organizational skills.
Empathetic and adaptable to diverse home environments and patient needs.
Reliable transportation for daily travel (up to 60 miles/day).
Monday Friday schedule; no weekends or on-call required.
Block scheduling or compressed workweeks (e.g. 4x10s 3x12s) not permitted.
Must comply with facilitys COVID-19 vaccination policy (primary series booster).
Facility reserves the right to cancel up to one shift per two-week pay period.
