100% day shift working Monday Friday 8:00 am 5:00 pm. will work at University Hospital and Eastpark Medical Center in Madison.

As an Advanced Practice Provider youll play an integral role in our ambulatory care team by providing high-quality cost-effective care to patients.

We are seeking a Nurse Practitioner or Physician Assistant to:

  • Work with a multi-disciplinary care team in Breast Medical Oncology.
  • Conduct comprehensive or episodic health history and physical assessment of Breast Cancer patients with acute chronic and/or potential health problems.
  • Oversee treatment infusions and supportive care needs.
  • Manage and modify treatment plans of patients as needed.
  • Provide ambulatory urgent call coverage for Breast Cancer patients as well as other oncology tumor types as needed.
  • Facilitate referrals procedures and inform ambulatory care coordination during discharge of hospitalized Breast Cancer patients

Education:

Minimum

If Nurse Practitioner: Masters or Doctorates degree from accredited Nursing Program.

If Physician Assistant: Bachelors or Masters degree from accredited PA Program.

Licenses and Certifications:

Minimum -

If Nurse Practitioner: Active RN and APNP license in State of Wisconsin AND active board certification as a NP in Adult; Adult/Gerontology Primary Care; Family; Gerontology.

If Physician Assistant: Active PA license in State of Wisconsin AND active PA board certification.

All Nurse Practitioner and Physician Assistant applicants must have: Active DEA license or ability to apply for such license prior to or upon hire AND CPR certification and advanced life support certifications if relevant to practice area (ACLS PALS NRP).
