Wellness Coordinator Senior Doc
Schedule: Full-timeOn-siteat multiple Senior Living Communities

In this roleyoullbe the communitys trusted point of contact forcare coordination wellness checks and resident engagement.Youllassistwith vitals telehealth appointments and communication between residents providers and families ensuring each senior feels supported and connected.
Key Responsibilities
Serve as theSenior Doc representativewithin the senior living community
Conductmonthly vitals and weightsfor residents
Supporttelehealth appointmentsandassistresidents with technology
Performdaily wellness checksand provide companionship
Coordinate communication between residents providers and families
Participate incommunity wellness and outreach events
Qualifications
High school diploma or GEDrequired
Medical Assistant experience or certification preferred
Prior experience insenior living patient care or care coordinationhighly valued
