Position Overview:The Manager of Social Services provides leadership for hospital-based social work programs overseeing discharge planning operations to ensure safe timely patient transitions and optimal continuity of care.
Key Responsibilities:
Lead supervise and develop the social work team and daily departmental operations.
Collaborate closely with case management nursing physicians and interdisciplinary teams.
Ensure adherence to regulatory standards and protection of patient rights.
Oversee discharge planning workflows and coordination of internal and community resources.
Advance process improvement patient satisfaction and care efficiency initiatives.
Qualifications:
Active LCSW or equivalent licensure.
Minimum 1 year of leadership experience in hospital social work or case management.
Previous acute care hospital experience required.
