We currently have an exciting opportunity for aComplex Needs Recovery Coordinator to join us. With this role youll be working as part of the Sunderland Altogether Improving Lives (SAIL) Team in central Sunderland.

The team supports people who may typically find it challenging to engage with treatment for drug and alcohol issues due to their complex needs. As a Complex Needs Recovery Coordinator you will support those people to engage with their Recovery Coordinator while working alongside other community services. Location: Sunderland

Hours: Full time 37.5 per week

Full Time Salary: 27861.26 - 32002.35 Dependent on experience (Based on full time hours pro rata for part time)

Contract Type: Fixed term for 9 months from start date

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Coordinate and leadasolution-basedapproach engaging Service Users inappropriate services
  • Carry a caseload of complex clients developing personalised action plans
  • Participate in multi-agency initiatives to reduce the level risk
  • Work closely with the drug and alcohol teamsto ensure that substance misusers and problem drinkers can accesstreatment
  • Work proactively to ensure that Service Users have access to ongoinghealthcare
  • To develop an Individual action Plan with each service user in conjunction with other agencies and professionals who will need to be involved.
  • To work with identified complex needs clients with substance misuse and mental health issues and physical issues to overcome barriers to services and engagement

About you:

  • Experience of working in one or more of the following fields: substance misuse alcohol misuse mental health
  • Experience of working with people who have multiple and complexneeds
  • Knowledge and experience of multi-agency workingwith experience offacilitatingmulti-agencymeetings
  • Excellent communicationnegotiationand advocacy skills
  • Be able to build andmaintainrelationships with a wide rangeofservices
  • Can work autonomously as well as part of ateam
  • Good levels of IT literacy including the ability to use IT systems including MSOfficeapplications
  • Able tocritically reflect and evaluate interventions service delivery and areasfordevelopment
