RESPONSIBILITIES
The licensed registered nurse provides skilled nursing care services by planning organizing directing coordinating and evaluating all aspects of nursing care in accordance with Physicians orders primarily delivered on a visiting and intermittent basis in the clients home in accord with the policies and procedures of the home health care agency. The Rn also teaches and supervises the family members to provide continuity of care and implement comprehensive care.
The Registered Nurse perform acts which require substantial specialized knowledge judgment and nursing skill based upon principles of psychological biological physical and social sciences. The Registered Nurse is responsible and account preparation and experience in nursing.
Ensures quality and safe delivery of home health care services:
- Completes nursing assessment forms in a timely manner per agency policy and procedure
- Identified physical emotional and developmental client needs.
- Develop implements and evaluated clients plans of care per agency policy and procedure.
- Develops individual nursing care plan which reflect current client needs prior to client case
conferences. - Client and Clients families are involved in developing clients plan of care whenever possible.
- Provides health teaching to client and clients family explaining the procedure to be done.
- Client clinical record documentation reflects delivery of quality and safe home health care
services. - Assesses clients condition every home visit
- Communicated client assessments to appropriate individuals as well as unusual reactions and changes in condition.
- Implements nursing interventions to meet client needs and changed conditions.
- Documents client clinical record per home health agency policy and procedure
- Accurately transcribes implements and evaluated physicians orders for clients in a timely manner.
- Refers clients to other health care providers per home health care agency policy and procedure
- Documents referral information accurately completely and in a timely manner.
- Implements infection control and safety measures per home health care agency policy and procedure.
- Implements clinical care per home health care agency policies and procedures.
- Visual/hearing ability sufficient to comprehend written/verbal
- Ability to perform tasks involving physical activity which may include heavy lifting extensive bending and standing.
- Ability to deal effectively with stress
