Job Summary
Travel RN Case Manager (Day Shift)
Location: Hyannis MA
Contract Duration: 13 Weeks
Shift: Rotating Days 8:00 AM 4:30 PM (every other weekend & rotating holidays)
Travel Pay: Up to $2686/week
Overview
Navitas Healthcare LLC is seeking Travel RN Case Managerfor an exciting job in Hyannis MA.
Responsibilities
Screen new admissions daily and identify discharge planning priorities.
Verify and coordinate patient demographic and insurance information with admissions staff.
Conduct initial and ongoing clinical assessments using approved guidelines and utilization tools.
Evaluate appropriateness of admission level of care and medical necessity; manage appeals and notifications as needed.
Address psychosocial financial and legal issues impacting care; connect patients with appropriate resources.
Oversee utilization review processes and coordinate with payers to support reimbursement.
Collaborate closely with physicians nursing staff and other care team members to ensure consistent patient management.
Act as a patient advocate promoting independence and informed decision-making.
Participate in interdisciplinary rounds case conferences and quality improvement initiatives.
Maintain compliance with all hospital policies documentation standards and regulatory requirements.
Qualifications
Minimum 3 years of recent acute care or case management experience (within the past 5 years).
Proficiency with InterQual or similar utilization management tools.
Recent (within 4 years) experience in discharge planning and utilization review.
Active Registered Nurse (RN) license in the state of Massachusetts (or compact state).
BSN preferred.
Certification in Case Management (e.g. CPUM CCM) preferred.
