Job Summary

ACO Triage Nurse

Millennium Physician Group

Full Time-Panama Shift 11am to 11pm includes every other weekend

Remote Work

Essential Duties and Responsibilities

Participate in the After-Hours On-Call Team rotation
Answer all incoming calls immediately and give correct advice
Provide Post-Acute Care Outreach
Assist Hospitalists Patient Outreach
Facilitate Prescription Refills
Assist RN Care Management Teams with patient assessments and/or monthly follow-up calls when needed
Monitor Patients enrolled in the Patient Monitoring Program

Required Knowledge and Experience

Registered Nurse license multi-state license required
License must always be kept current
5 Years of clinical experience with 1 year(s) of case management experience preferred
ER/ICU/Triage experience is preferred.
RN ED experience preferred
