The Registered Nurse Case Manager provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Effective communication and collaboration with patients providers and team members are essential.
The Registered Nurse Case Manager provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Location:Torrance California United States
Responsibilities:
- Conduct patient assessments to determine care needs.
- Administer medications and treatments as prescribed.
- Develop and implement patient treatment plans.
- Collaborate with an interdisciplinary team to coordinate patient care.
- Provide skilled nursing care in various settings including medical/surgical ICU ER psychiatric labor & delivery PACU and operating room.
- Maintain accurate and timely documentation of patient care.

Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Bachelors degree in Nursing or related field.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Minimum of two years of hospital home health or hospice care experience.
- Drivers license car insurance and automobile for field work.
- Active Registered Nurse license in the state of working.
- Bachelors degree in Nursing or related field.
