The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with an interdisciplinary team. Strong communication and collaboration skills are essential for effective interaction with patients providers and team members.

Required Skills & Certifications:
- Active Registered Nurse license in the state of working
- ACLS certification
- BLS certification
- Clinical Assessment Medication Administration and Treatment Planning skills
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
- Minimum 2 years of hospital home health or hospice care experience
- Trauma II experience
Preferred Skills & Certifications:
- Not specified
Special Considerations:
- May float between like units
- Facility is able to call off once per two week pay period
Scheduling:
- Weekend requirement: Every other weekend
- Open to accommodating block schedule
- First-timers accepted
- RTO must be included at time of submission
- On-call is not required
