Home Health Physical Therapist (PT)

What Youll Do
Deliver hands-on personalized therapy that helps patients regain strength and mobility.
Collaborate with care partners to design treatment plans that fit each persons goals.
Track and celebrate progress with patients and families.
Keep accurate documentation and clear communication across the care team.
Share your expertise and promote physical therapy in the community.
What Youll Bring
Graduate of an APTA-accredited Physical Therapy program
Current unrestricted PT license in Michigan
At least 1 year of experience in a healthcare setting (home health preferred)
Reliable transportation valid drivers license and auto insurance
Ability to travel up to 50% within Hillsdale County
Physical ability to assist and support patients (50100 lbs)
