Job Summary
Social Worker - Montrose Find your life s work at HopeWest as Social Worker supporting our patients and families in Montrose County. You will be part of an interdisciplinary care team of social workers chaplains nurses physicians and certified nursing assistants all working together to improve people s everyday lives when they need help the most. You will be an advocate by providing education and resources to patients and their families being out on the road and seeing our patients/families in their personal living environments in this beautiful rural community. The Social Worker will work with an exemplary and robust interdisciplinary team focused on providing quality care to patients in the community through education resources and care that honors the goals and wishes of each person. Who Should Apply Social Workers with a minimum of two years experience in healthcare. Experience working (paid or volunteer) with the elderly preferred. Excellent interpersonal and communication skills. The Job Location:Across Montrose county: HopeWest Social Workers provide care in patients home settings.
