Care Management for At-Risk Children (CMARC) is an at-risk population management program that serves children from birth to 5 years of age who meet certain risk criteria. The care manager evaluates and develops a plan of care based on risk factor criteria and a psychosocial assessment. The care manager works to coordinate services within the community to support the childs developmental social emotional and physical well-being. The services coordinated through this program include primary and specialty health care special therapies developmental screening physical assessments and services to address social determinants that impact the childs development and overall health.Contacts with the child and their caregivers may occur in multiple settings including the health care provider office community setting or patients home as well as by phone or video conference. CMARC care managers work with other professionals in the Clinical Services Division in the delivery of appropriate services. The care manager assumes responsibility and accountability for the practice of professional care management in a manner consistent with the standards set by the agency. Top candidates are self-directed and can work autonomously following all policies and program guidelines once training addition candidates must be organized able to multitask and must have the ability to communicate effectively with a variety of clients.

Typical Qualifications

MINIMUM EDUCATION AND EXPERIENCE:Bachelors degree from an accredited school of social work and one year of social work or counseling experience; OR Masters degree from an accredited school of social work and one year of social work or counseling experience. Pediatric experience preferred but not required.


WORK SCHEDULE: This is a full-time position and primary work hours are Monday Friday 8 a.m. to 5 p.m. The primary work location is the Mooresville office of the Iredell County Health Department.

SALARY RANGE: $54587.46 - $85156.43 (Grade 329 C)
