Chaplain

Primarily responsible for the provision of spiritual care services to meet the social emotional and spiritual

needs of patients and families/caregivers either directly or through coordination of care with other spiritual

counselors.

• Facilitates the development of the interdisciplinary team plan of care to meet individual spiritual needs.
• Maintain record of spiritual care services and related activities for quality assurance program development and policies and procedures.
• Document direct services and ongoing communication with community chaplains spiritual counselors and hospice interdisciplinary team.
• Attend Interdisciplinary Team meetings and complete all associated documentation in an accurate and timely manner.
• Provide consultation education and support to the interdisciplinary group on spiritual care.
• Meets measures as determined and required for job productivity and performance improvement.
• Provide bereavement follow up services as requested.
• Arrange for on-call availability of chaplaincy services.
• Conduct or make arrangements for funeral or memorial services for patients as requested.
• Assist in planning periodic memorial services to meet the needs of personnel volunteers and community clergy/spiritual counselors working with the hospice team.

Education:

• Graduate of accredited school of theology or related Bachelors degree.
• 1 unit of CPE at time of hire. 2nd unit of CPE must be completed within 1 year of hire.

Experience:

• Ordination in a particular religious group and/or endorsed and in good standing from an ecclesiastical governing body

Licensure:

• Valid drivers license when driving any vehicle for work-related reasons.
• Valid Mandatory Reporter course completion by state(s) requirement
• CPR Certification.
