Job Summary

Admission RN Home Health (Monongahela PA)

Full-time Estimated Salary: $80000 per year

This position offers a supportive work environment competitive pay and opportunities for professional growth. The Admission RN will manage and deliver quality patient care while coordinating with the interdisciplinary team to ensure positive outcomes.

Responsibilities:
Manage and deliver individualized patient care plans
Collaborate effectively with the care team to ensure seamless service delivery
Provide comprehensive and compassionate patient care in the home setting
Complete documentation accurately and meet charting deadlines
Maintain effective time management and scheduling
Utilize mobile devices and EMR systems for care coordination

Schedule:
Monday through Friday 8-hour shifts
One evening on-call per week (5 PM 10 PM)
One weekend per month (Saturday/Sunday)

Qualifications:
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of experience using EMR systems
Valid drivers license
Current BLS and/or ACLS certification through the American Heart Association
