Job Summary:
This Physical Therapist position involves a combined outpatient and home health model of care focusing on the geriatric population. The Physical Therapist will conduct PT sessions initial assessments and evaluations working closely with an interdisciplinary team to promote safe mobility for aging patients in various settings including homes assisted living facilities skilled nursing facilities and hospitals.
Location:Seattle Washington United States
Responsibilities:
- Conduct physical therapy sessions with participants at the center or their place of residence.
- Perform initial 180-day and change of status assessments.
- Evaluate and order Durable Medical Equipment (DME).
- Collaborate with families Assisted Living Facilities (AFHs) Assisted Living Facilities (ALFs) Skilled Nursing Facilities (SNFs) and hospitals to assess and promote safe mobility.
- Develop and implement independent care plans around physical therapy.
- Work closely with the Interdisciplinary Team (IDT) regarding participant care.
- Attend morning meetings rehab meetings and care conferences as needed.
- Collaborate with the rehab unit at the center to prevent gaps in care.
- Document patient care and related duties appropriately.
Required Skills & Certifications:
- Licensed Physical Therapist in WA state.
- Minimum 1 year of experience.
- Experience with Epic.
Preferred Skills & Certifications:
- Not specified.
Special Considerations:
- Clinician may float within scope to meet facility needs.
- No weekends nights or major holidays.
Scheduling:
- 5x8 day shifts.
