In Hexie VA we are looking for a Medical Summary Reviewer your primary responsibility will be to create accurate and consistent medical summaries for the Special Damages Chart. You will review medical records from various providers following the SOAP format (Subjective Objective Assessment Plan) to ensure comprehensive documentation.
Key Responsibilities:
o Review Medical Records: Examine records to identify the patients
presenting symptoms specific injuries and detailed descriptions of
pain (e.g. left vs. right side).
o Summarize Patient Information: Start each summary with Patient
presents with following a and include descriptive symptoms
such as sleep issues concentration difficulties and numbness.
o Document Examinations: Note each examination performed and
the corresponding findings.
o Assessment and Diagnosis: Record the diagnosis using medical
codes or listed terms.
o Plan and Follow-Up: Document the treatment plan including follow-
up schedules. If no plan is provided note No plan given or patient
will return as scheduled.
Additional Duties:
o Record Review: Ensure the medical providers name is correctly
spelled and matches throughout the chart.
o Date of Service: Use the actual visit date for records not the date
when the record was dictated.
o Charge Documentation: List charges for services accurately
excluding fees record requests or insurance dates.
Required Summaries:
o SOAP Format: Required for the first and last Date of Service (DOS)
for providers such as Physical Therapy Chiropractic Care
Massage Therapy Mental Health Speech Therapy Occupational
Therapy Home Health and Rehabilitation.
o Charge Listing: For providers like Radiologists Emergency
Physicians Laboratory Anesthesia and Surgeons only list
charges without summaries.

monthly quota for reviewing clients bills contributing to the teams overall
performance metrics and ensuring timely completion of tasks.

records. Medical Terminology: you will be learning and using medical terminology
regularly.

What Youll Need
Health Information Management: A background in reviewing or dealing
with medical records is preferred but not required.
Self-Starter: Proven ability to initiate and complete tasks independently
without constant supervision.
Attention to Detail: Meticulously review documents and identify relevant
information.
Software Proficiency: Utilize document review software effectively.
Effective Communication: Collaborate with colleagues and attorneys.
Organizational Abilities: Manage tasks efficiently.
Required Skills:
Bachelors degree in Law Legal Studies or a related field. Proven experience as a paralegal or legal assistant preferably with a U.S.-based law firm. Strong understanding of U.S. legal systems processes and terminology. Proficient in legal research tools (e.g. Westlaw LexisNexis) and office software (e.g. Microsoft Office Google Workspace). Highly organized with strong attention to detail and ability to manage multiple tasks simultaneously. Reliable internet connection and ability to work U.S. business hours. Advanced level of english is mandatory Have a laptop/desktop and headphones to do the job Steady internet connection Previous experience in a similar field in english will be highly considered but not mandatory 100% remote work
