Job Description

JOB TITLE: Bereavement Coordinator

REPORTS TO: Director of Nursing

JOB SUMMARY: The Bereavement Coordinator is responsible for developing a comprehensive bereavement program and for ensuring that patients and their families receive any bereavement support that may be needed.

JOB SPECIFIC RESPONSIBILITIES:

  1. Upon patients admission reviews the Plan of Care with the Hospice Interdisciplinary Group (IDG) to assess bereavement risk and need for services.

  2. Offer support and information on grief and loss to patients and family.

  3. Upon a patients death develops a bereavement Plan of Care based on the familys needs and contact within 48 hours of the patients death.

  4. Initiate contact with families prior to death when the patient is determined imminent.

  5. Ensures that the bereavement Plan of Care is followed and provides ongoing bereavement assessment.

  6. Assumes and maintains accountability for bereavement program compliance with federal and state regulations.

  7. Facilitates or participates in grief support groups.

  8. Completes all required documentation.

  9. Participates in IDG meetings.

  10. Facilitates memorial service for bereaved families at least annually.

  11. Participates in CQI activities.

  12. Provides bereavement training for volunteers.

  13. Assists in the admission process as required.

  14. Participates in staff training and education programs.

  15. Participates in an on-call rotation.

MINIMUM REQUIREMENTS:

  1. Bachelors degree in psychology counseling pastoral ministry social work sociology or related field would be preferred but not required.

  2. Previous training and/or experience in death and loss issues.

  3. Knowledgeable in cultural ethnic and spiritual diversity.

  4. Ability to work well independently and within the interdisciplinary team setting.

  5. Demonstrated excellent oral written and interpersonal communication skills. Ability to socially interact professionally with the appropriate attitude.

  6. Excellent organizational skills

  7. Ability to work a flexible schedule.

SALARY LEVEL:

non-exempt

PHYSICAL AND SENSORY REQUIREMENTS:

  1. Ability to read write and converse in English;

  2. Ability to communicate with staff residents families vendors support agencies and others.

  3. Ability to remain calm and composed during stress.

  4. Bending lifting grasping fine hand/eye coordination pushing/pulling prolonged sitting/standing.

  5. Ability to respond to auditory stimulation.

  6. Mathematical ability (addition subtraction multiplication division standard measurements).

  7. Ability to evaluate/interpret information and make independent judgements/decisions within job position parameters.
