Registered Nurse

The Atlanta Regional Commission (ARC) is the regional planning and intergovernmental coordination agency that focuses on issues critical to the regions success including growth and development transportation water resources services for older adults and workforce solutions. The Registered Nurse is responsible for providing clinical support and consultation to intake staff including completing Level of Care (LOC) reviews for individuals being screened for and referred to the Elderly and Disabled Medicaid Waiver Program (EDWP) to ensure accurate assessment and referral of individuals seeking services. This position involves conducting telephone and face-to-face intake and screening identifying service needs of older adults individuals with disabilities of all ages caregivers and members of the public seeking information about both public and private long-term care options and other available resources. The RN will enter screening data notify clients of referral outcomes and perform comprehensive needs assessments. The RN will also participate in special projects and contribute to the development of referral policies and procedures create resource materials and training tools generate customized client reports collect and analyze outcome data support quality assurance initiatives serve as a liaison to community organizations and service providers and perform other duties as assigned by the supervisor.

In addition to clinical responsibilities this position functions within the framework of the Aging and Disability Resource Connection (ADRC) assisting individuals across the Atlanta region in accessing community-based support and services. This includes providing information and referral counseling intake and screening for the Elderly and Disabled Waiver Program and the Home and Community-Based Services Program and determining service eligibility and needs.

This role is fully remote after the introductory period. There are occasionally times that this position requires coming into the office. Individual must reside in Georgia.

• Provides Level of Care (LOC) reviews on individuals being screened/referred to EDWP.
• Provides nursing consultation to intake staff on clinical issues.
• Provides telephone or face-to-face consultation to determine program eligibility; assesses the needs of clients using a person-centered approach to determine resource options; and performs intake and screening to determine program eligibility.
• Notifies clients of referral status.
• Informs applicants of appeal rights.
• Documents client interactions in client management system including assessment identified needs referrals and outcome.
• Conducts re-screening of clients on waiting lists to certify continued need for services.
• Conducts need assessments; develops scope of services; develops policies and procedures; identifies and develops needs assessment and care planning tools; researches and creates resource materials;
• Develops outcome measures for service/program evaluation; collects data; assists with database development.
• Conducts quality control oversight of contract providers engaged in client care assessment.
• Develops and provides training to staff members subscribers stakeholders and other community agencies.
• Participates in special projects that may include (but are not limited to) promoting and marketing services community outreach.
• Collaborates and engages with community partners to facilitate ease of access to service.
• Conducts follow-up with clients to ensure resources are received are appropriate and individual outcomes are achieved.
• Performs other duties assigned to support agency goals and objectives.

• Completion of a Registered Nurse program and two years of clinical nursing experience

• State of Georgia Registered Nurse License.
• Meets state requirements to become a certified Inform USA Specialist within one year of employment and maintain certification annually (by position assignment).

• Ability to perform basic original research compile and assemble data.
• Ability to prepare basic reports and documents from established information or by following a standard format.
• Ability to monitor program quality and evaluate contract providers.
• Ability to maintain and update data and documentation.
• Skill in comprehending reference books and manuals.
• Skill in conducting and completing telephonic intake and client needs assessments.
• Skill in determining client level of impairment and unmet needs.
• Skill in determining application program eligibility or priority for referral.
• Skill in organizing tasks meeting deadlines and prioritizing competing demands.

• Possess exceptional time management interpersonal and communication skills.

• Proficient in database technology and Microsoft Office Suite (Outlook Word Excel Teams and PowerPoint).
