Community Links health team  is embarking on a new chapter with funding secured to deliver an innovative 12-month pilot aimed at tackling health inequalities across London. As an Outreach Health Navigator you will play a pivotal role in the Community Health Bank project engaging with food bank users and other vulnerable communities in Newham to improve access to preventative health services and address disparities in health outcomes.

This is a frontline role where your engagement can make a real difference. You will provide advice guidance and support to help people overcome barriers to healthcare while also contributing to the monitoring and evaluation of the project alongside the Project Coordinator and Programme Manager. Be part of a team dedicated to creating healthier stronger communities and make an impact that truly matters.
