In this vital role youll be at the heart of a multidisciplinary team ensuring that each patients journey is met with dignity comfort and clinical excellence. Youll coordinate and deliver holistic care plans advocate for patients needs and support families through one of lifes most profound transitions. Responsibilities
Conduct timely and comprehensive assessments of patients physical mental emotional functional financial and spiritual needs.
Provide skilled nursing care aligned with the physician/APC plan of care.
Evaluate patient responses and adjust interventions as needed in collaboration with the care team.
Develop and implement individualized hospice care plans with input from patients caregivers physicians and the care team.
Facilitate care delivery using community resources and interdisciplinary collaboration.
Ensure care plans are followed and updated in compliance with regulatory standards.
Maintain accurate timely and complete documentation to support quality care and reimbursement.
Ensure all records reflect care provided and are accessible to the care team per agency policies.
Precept newly hired nurses and students serving as a professional role model and resource.
Delegate appropriate interventions to LPNs and CNAs ensuring quality and consistency.
Uphold current nursing standards and best practices.
Participate in Quality Assurance Performance Improvement (QAPI) and Performance Improvement Projects (PIPs).
Assume leadership roles and contribute to departmental initiatives as needed.
Maintain open timely communication with the multidisciplinary team.
Advocate for patient needs and ensure care plan changes are communicated and implemented efficiently.
Provide holiday and weekend coverage including potential for both day and night shifts.
Perform other duties as assigned to support patient care and team operations.
Qualifications
- Associate Degree in Nursing
- State of New Mexico or Compact State Nursing License
- BLS certification REQUIRED at time of hire
- 1 year of direct patient care setting or home health/hospice care
- Current drivers license and auto insurance.
- Basic typing competency.
