Key Responsibilities:
- Work and liase with Social Inclusion Team International Protection Applicant Services staff
community Health Networks community services and NGOs supported by the Social
Inclusion Team to ensure and promote integrated and optimum healthcare access.
- Work as part of a multidisciplinary team to meet the needs clients and their families
advocating on their behalf. Liase with social care team and outreach team to provide
holistic care and meet the nursing needs of individuals and their families.
- Promote patient care and independence by seeking to integrate patients and link to support
groups and appropriate healthcare pathways

Requirements
- Must be registered with NMBI (Nursing and Midwifery Board of Ireland)
- Minimum of 6 months post-registration experience
- Access to own car and full licence is essential for this role
