Role Summary:

  • Investigates assesses and plans interventions to help patients cope with social emotional economic and environmental problems
  • Provide individual and group counseling to cancer patients and their families
  • Assist patients in navigating the healthcare system and accessing necessary resources
  • Conduct psychosocial assessments and develop care plans make internal and external referrals as needed
  • Coordinate with community resources and support services such as financial assistance programsassist with the coordination of prescription benefits and finding financial support for oral chemotherapy medications.
  • Lead support groups and educational workshops on coping strategies and stress management
  • Provide grief and bereavement counseling
  • Develop and implement programs to address specific needs of oncology patients

Qualifications:

  • Education:Masters degree
  • Licenses:State of Montana LMSW
  • Experience: 2 years related experience
  • Certifications:Basic Life Support (BLS-HSCP)

Essential Functions

  • Interviews and assesses patients and/or patients family caregiver and/or legal representatives.
  • Determines prioritizes provides and/or arranges for needed internal and external services/interventions.
  • Participates in case reviews to evaluate case management and progress.
  • Consults with healthcare team members to promote monitor and evaluate compliance with patients treatment plan.
  • Assists with discharge planning and processing.
  • Identifies appropriate resources including transportation housing healthcare and social/spiritual services and provides referrals as part of discharge plan.
