Schedule: Full Time 40 hours/wk. Monday-Friday 8 am - 4:30 pm. No weekends holidays or call.

Role Summary:
- Investigates assesses and plans interventions to help patients cope with social emotional economic and environmental problems
- Provide individual and group counseling to cancer patients and their families
- Assist patients in navigating the healthcare system and accessing necessary resources
- Conduct psychosocial assessments and develop care plans make internal and external referrals as needed
- Coordinate with community resources and support services such as financial assistance programsassist with the coordination of prescription benefits and finding financial support for oral chemotherapy medications.
- Lead support groups and educational workshops on coping strategies and stress management
- Provide grief and bereavement counseling
- Develop and implement programs to address specific needs of oncology patients
Qualifications:
- Education:Masters degree
- Licenses:State of Montana LMSW
- Experience: 2 years related experience
- Certifications:Basic Life Support (BLS-HSCP)
Essential Functions
- Interviews and assesses patients and/or patients family caregiver and/or legal representatives.
- Determines prioritizes provides and/or arranges for needed internal and external services/interventions.
- Participates in case reviews to evaluate case management and progress.
- Consults with healthcare team members to promote monitor and evaluate compliance with patients treatment plan.
- Assists with discharge planning and processing.
- Identifies appropriate resources including transportation housing healthcare and social/spiritual services and provides referrals as part of discharge plan.
