The MDS Coordinator coordinates and completes documentation requirements in the nursing department in accordance with current federal state and facility standards guidelines and regulations. Routinely contacts staff families other department directors/coordinators and physicians as needed.
Ideal Candidate
- Successful completion of an approved accredited nurse education program required.
- Experience in Long Term Care is preferred.
- Minimum Data Set experience preferred
Essential Functions:
- Serves as the Minimum Data Set (MDS) Coordinator for the campus functioning to coordinate and complete the entire
- MDS process working with other departments as needed.
- Assesses residents health status needs through physical assessments observation and interview and documents findings accordingly.
- Provides and monitors accurate documentation in the resident record according to established nursing policies procedures and standards. Acts as a resource for accurate and complete documentation to include Medicare and case mix classifications.
- Completes monitors and evaluates residents plans of care making appropriate changes in plans of care whenever necessary. Ensure that plans of care are complete and comprehensive.
- Maintains proper documentation of all Medicare MSHO (if applicable) and insurance information on residents/tenants and helps determine Medicare eligibility. Monitors all staff to maintain necessary records for Medicare. Works with Resident Care Managers as needed for insurance products.
- Provides education to nursing department staff as needed including but not limited to nursing orientation computer programming training and documentation techniques.
- Provides case mix and Medicare information to residents and families as needed.
- Issues notices of Medicare non-coverage (if applicable).
- Assesses educational needs of unit staff and provides input to the Director of Nursing.
- Assumes the responsibilities of committee membership as assigned by the Director of Nursing.
- Will electronically submit MDS data per regulatory requirements. Will run quality reports and other reports as needed.
- Helps develop and/or update nursing policies and procedures as needed.
- Refers problems of an unusual or complex nature along with recommended solutions to the Director of Nursing.
- Follows all resident care policies and procedures and complies with rules and regulations set forth by the Department of
- Health and CMS.
- Continually updates nursing skills and knowledge by attending in services continuing education programs and staff meetings.
- Will rotate weekend and weekly on call responsibilities and provide leadership to direct care staff.
Certificates Licenses Or Registrations:
Must have a NE RN License.
Must currently be in good standing in all states in which you have ever been license as a nurse in the U.S.
Current Certificate in CPR/BLS is required.
