About the role
This a hybrid role with duties shared between our Family Connect and Support Service and Family Support Service. This role brings a case management or case coordination approach to bringing families together and supporting them to increase their capacity and functioning through direct case management or case corordination referral to support services and community resources to prevent a difficult situation from escalating to a crisis requiring a statutory response from the child protection system.
As our Case Worker you will undertake an initial assessment of family strengths and needs provide family support through case planning home visiting and referral planning. You will provide direct support for the family to help them achieve their goals make appropriate referrals to other agencies and follow up with services to ensure successful engagement. You will also provide information and advice to community members and services and facilitate parenting groups when required. Typically we engage with families on a short-term basis (up to six months).
