Job Summary

OCES supports elders and individuals with disabilities by providing vital information and coordination of services. By promoting healthy save lives for our consumers we hope to allow them to stay as active and engaged in their lives and in their own communities for as long as possible.

To that end we are seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries.

What you will do:

  • Assess the health and functional status of members.
  • Develop community-based care plans and related service packages.
  • Participate in the development of a members Individual Care Plan (ICP)
  • Arrange coordinate and monitor the provision of appropriate community services.
  • Promote the independent functioning of members.
  • Participate in the determination of a members need for institutional and community long term care services.

What you will bring:

  • Your passion for helping others and desire to make a difference.
  • Bachelors degree in social work or related field
  • Minimum of two years professional experience in the care of persons 65 and older.
  • Current Social Work license (LSW or higher) preferred.
  • Working knowledge of the social emotional and physical processes of aging
  • Ability to gain and hold the confidence of clients and their families.
  • Ability to obtain information of a personal nature through interviews.
  • Draw appropriate conclusions and make recommendations.
  • Knowledge and skill necessary to work successfully within an interdisciplinary team; ability to express clearly both orally and in writing
