The District Nurse will provide high quality nursing services including assessment of broader care needs to a diverse community. The District Nurse is responsible for taking a wellbeing and reablement approach in improving the health status and independence of the client and supporting the carer in their caring role
Classification for this position will be YU1 ($107541.20 per annum pro rata).

About St Vincents At Home District Nursing Team
This position is part of our St Vincents Virtual & Home program which provides the opportunity for more people to receive high quality clinical care at home. Integrated with St Vincents Health Australia hospital and community care network we are a multidisciplinary team of physicians allied health nursing and personal care workers supporting individuals to leave hospital sooner remain in their homes and achieve their goals. We are an experienced supportive and dynamic team committed to the growth of quality care within the community.
The District Nursing team provides community nursing care to clients under the Commonwealth Home Support Program (CHSP) Victorian Home and Community Care-Program for Younger People (HACC-PYP) Transition Care Program and is a service provider to Department of Veteran Affairs (DVA) eligible clients. The District nursing service is also responsible for our Home Based Cancer Care pathways as part of our Better @ Home funding a key pillar of our health service Care Beyond the Walls strategy.
