Salary: $17.98 - $21.76 hourly (3 levels based on experience and education)

Job Summary
The Success Coach (Case Manager) for Targeted Case Management (TCM) will provide specialized case management services to individual client needs. They also create programs to benefit clients and educate clients on available community resources. Success coaching ensures that clients receive services in a timely appropriate efficient and coordinated fashion.
This position requires that in-person support and/or service delivery be provided by the employee in various settings approved by organizational leadership to include but not limited to: homes schools outdoor settings courtrooms offices shared spaces street outreach locations etc. These environments are often inclusive of higher levels of noise and other factors that may present a heightened level of stimuli and safety awareness. When providing or participating in leadership-approved on-line activities being on-camera and actively engaged are also expected and required as part of this work.
What Youll Do:
- Coordinate services between providers and participants
- Provide case management services and skill-building services to youth in the program
- Provide housing case management services
- Maintain individual case records on participants receiving services
- Provide specialized case management services to individual participant needs
- Educate participants on available community resources and services
- Create programs and services to benefit clients and the community
- Attend community meetings relevant to program and clients
- Administer and facilitate the completion of appropriate intake forms assessments intake packet and service plans
- Conduct field visits attend meetings and be available to deliver services during traditional and non-traditional hours (when needed) in educational institutions places of employment FYIs main office JPPO offices detention facilities or any other setting that enables and promotes the consistent and structured delivery of case management services
- Organize and facilitate life skills in both individual and group settings
- Required to participate and interact in youth activities (for example: camping sport activities overnight
youth conferences etc.) which may involve occasional overnight activities and trips
- Must pass a CYFD Criminal Records Check
- Must maintain a full certification of the following trainings; CPR/First Aid and CPI
What You Need:
- Bachelors degree in Social Work Family and Consumer Sciences Criminal Justice Psychology Sociology or Criminology from an accredited College or University.
- Four (2) years experience working with at risk youth or children in an educational behavioral health or similar setting.
- Excellent communication skills both written and verbal
- Exhibit skills in comprehending interpreting and completing delegated tasks efficiently
