Job Summary
Hospice Social Worker
Location: Jackson MS
Job Type: Hourly Non-Exempt

We are seeking a skilled and empathetic Social Worker (SW) to join our this vital role youll help patients and families cope with the emotional social and practical challenges that come with a terminal diagnosisbringing comfort and clarity during their most vulnerable moments.
What Youll Do
- Conduct psychosocial assessments of patients and caregivers
- Provide counseling to patients and families regarding emotional stress coping grief and advance care planning
- Assist with advance directives funeral arrangements and navigating legal/financial decisions
- Identify and coordinate with community resources agencies and support organizations
- Educate staff and families about cultural and psychosocial needs
- Document patient conditions interventions and psychosocial support provided
- Serve as a liaison between families and care teams to ensure coordinated holistic care
- Participate in interdisciplinary team meetings to develop individualized care plans
- Provide crisis intervention and support for safety concerns (neglect abuse self-harm)
- Participate in quality improvement initiatives and continuing education
- Participate in on-call support as needed
Qualifications
- Masters degree (LMSW) or LCSW required
- At least 1 year of experience in social work preferably in hospice home health or a community setting
- Knowledge of end-of-life care grief support and cultural sensitivity
- Strong written and verbal communication skills
- Self-motivated organized and able to work independently
- Current Basic Life Support (CPR) certification
- Valid drivers license auto insurance and reliable vehicle required
