The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other team members are critical to this role.

Responsibilities:
- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management.
- Provide hospice care.
- Communicate effectively with patients providers and team members.

Required Skills & Certifications:
- Active Registered Nurse license.
- ACLS certification.
- BLS certification.
- Minimum 5 years of community hospital experience.
- Scrub experience.
- Experience with Epic charting.
