Job Title: Nurse Case Manager
Ministry/Facility: Trinity Health PACE- St Joseph
Position Purpose:
The Nurse Case Manager coordinates and evaluates health care and social services for participants in the PACE program. Acting as a liaison between the interdisciplinary team (IDT) participants caregivers and community agencies this role ensures seamless care delivery and support.
What You Will Do:
- Lead and collaborate with the IDT to develop and implement individualized care plans.
- Conduct assessments home visits and hospital discharge coordination.
- Maintain accurate documentation and communicate effectively with participants and families.
- Mentor staff manage departmental operations and participate in quality improvement initiatives.
- Ensure compliance with federal state and organizational regulations.
Minimum Qualifications:
- RN license in the state of the PACE organization; BSN preferred.
- CPR certification required.
- 2 years of experience in geriatric care; 1 year working with frail or elderly populations.
- Valid drivers license and insurance.
- Strong clinical leadership and communication skills.
