Tribute Home Care is seeking a customer-service-focused dedicated and collaborative Case Manager to join our Massachusetts team. This role is ideal for someone with experience in client management customer success case management or account management who is eager to bring their expertise into a mission-driven healthcare setting.


While prior clinical experience is welcome it is not required. We believe candidates with strong communication organizational and relationship-building skills from non-clinical professional environments can excel in this role. What matters most is your eagerness to learn compassion for those receiving care and the incredible people that provide it and a commitment to providing exceptional service.

A day in the life of a Case Manager

In this role you will work with prospective and current clients their families and other professionals involved in their care to ensure they have a positive experience from the start of their home care services.


This will be done by:

  • Providing clients with a warm and helpful introduction to Tribute through responding promptly to inquiries for new service and setting up and completing in-person assessments
  • Overseeing the setup of Tributes services through formulating client care plans helping successfully match Caregiver Associates with clients and orienting caregivers to clients care plans
  • Providing regular proactive communication to all parties involved in each clients care
  • Revising care plans as often as necessary
  • Developing strong and trusting relationships with Tributes clients and their families

In addition to building relationships with clients you will also build relationships with community referral sources. We are seeking a candidate who is creative and resourceful and is eager to promote and grow a service they will be proud of.


This role will involve regular travel in the Boston and MetroWest Boston areas to assess and visit clients.

This person has the flexibility to work from home when not visiting or assessing clients meeting with referral sources or attending in-person meetings at our Framingham office 1-2 times a month.


Heres what the ideal candidate will bring to Tribute

  • Bachelors degree from an accredited college
  • Prior experience in caseload management customer success account management or a related client-facing role
  • An eagerness to impact growth
  • Unwavering passionate commitment to the customer experience and service excellence
  • Strong writing and communication skills
  • Strong attention to detail and organization
  • Collaborative spirit
  • The ability to travel within the Boston and MetroWest Boston areas regularly to assess and visit clients
  • The ability to travel to Tributes office in Framingham and to in-person events within our Massachusetts service area 1-2 times a month
