Hiring: Physical Therapist
Location: Rutland VT 05701
Schedule: M - F 8am to 4pm
Weekly Gross: $2800/week - $3000/week
Responsibilities:
- Actively monitors and amends patient visit schedule to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedule including but not limited to calling patient to reschedule visits reshuffling patient load to meet new demands communicating recommended changes to supervisor etc.
- Performs a comprehensive (OASIS and PT) assessments of assigned patients including but not limited to psychosocial educational safety and risk factor parameters as evidenced by clinical documentation and in accordance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines.
- Develops a plan of care to promote optimal functional independence and safety as documented by clinical record in accordance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines. Creates patient specific goals and ensures that the plan of care reflects logical interventions and advancement towards these goals.
- Plans for and provides skilled physical therapy treatment including but not limited to therapeutic techniques patient/care giver education etc. assessing the patients status and progress on each visit and ensuring quality of care as documented in the clinical record and in accordance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines.
- As Care Manager and/or team member coordinates patient care with other clinical team members including but not limited to physicians nurses care givers other rehab services and students ensuring optimal patient care and communication as noted in the clinical record and in compliance with federal and state regulations organizational and departmental policies and standards of care and APTA guidelines.
- Supervises and directs clinical activities of PTAs PT students and home care aides in accordance with federal and state regulations department and organizational policies and standards of care and APTA guidelines.
- Completes and coordinates required paperwork in accordance with federal and state regulations organizational and departmental policies standards of care and APTA guidelines to ensure that documentation reflects care provided and the outcomes achieved as evidenced in the clinical record..
License and Certification:
- Active PT license
- BLS -AHA
