We are seeking a compassionate and skilled Speech-Language Therapist to join our interdisciplinary team. This role involves evaluating and treating patients with speech language communication cognition hearing and swallowing (dysphagia) disorders across a variety of care settings.

Responsibilities
Evaluates and treats patients regarding the application of a wide variety of therapeutic interventions for rehabilitation of speech language communication cognition hearing and dysphasia disorders.
Must demonstrate knowledge of the principles of growth and development over the life span of patients served
Oversees and insures a comprehensive rehabilitation program designed to meet the physical psychosocial cognitive and immediate environmental well-being of the patient.
Promotes patient/caregiver self-determination by optimizing outside resource utilization to achieve the highest level of independence.
Coordinates and communicates with various disciplines or services involved in patient care.
Coordinates efforts toward discharge goals established at initial evaluation and during episode of care.
Is a patient advocate.
Other duties as assigned.
Settings may include inpatient outpatient long term care community based care and home health care
Requires detailed knowledge of speech and language pathology in terms of what and how work is to be done as well as why it is done this level includes interpretation of data
Basic understanding of the rules and regulations of the specific setting
Qualifications
- Masters degree in Speech and Language Pathology
- Certificate of Clinical Competence
- 1-3 years experience or Clinical Fellowship eligible
- Speech Language Pathologist
- BLS Required at time of Hire
- Must meet all the educational requirements of Home Health Medicare Conditions of Participation if necessary.
