Position Overview:
We are seeking a compassionate and community-focused Registered Nurse to conduct home visits for lead testing and case management. This role is ideal for an RN who enjoys community outreach and making a direct impact on public health.

Qualifications:
- Education:Graduate of an accredited nursing program.
- Licensure:Active Registered Nurse (RN) license in the state of Kentucky. CPR certification required.
- Experience:Experience in home visitation is preferred.
- Technical Skills:Proficiency in phlebotomy (blood draws) and accurate clinical documentation.
- Soft Skills:Exceptional interpersonal organizational and communication skills. Comfort with community outreach is essential.
Key Responsibilities:
- Conduct in-home blood draws for lead testing.
- Provide education and follow-up case management.
- Assist with special public health projects.
- Document care accurately in patient records.
