About the Role
The Home Team is an interprofessional team designed to support discharge from hospital or prevent unnecessary hospital admission through rapid allied health and nursing assessment and support at home.
This service works in partnership with clients key stakeholders and uses frailty as an indicator. The Home team play a critical role in supporting the Medical Assessment unit. This is a fast-paced work environment where patients admitted from ED or direct from the community are comprehensively assessed to determine if ongoing inpatient care is required or if the patient can be discharged home.

This role belongs to the DHBs/PSA Allied Public Health and Technical Multi-Employer Collective Agreement and falls between Step 1$77087 and Step 7 $107600 p.a pro rata of the Degree-based Allied Public Health and Technical Salary Scale.
