Patient Care Manager- Home Health
Location: Philadelphia & Elizabethtown PA
Type: Direct Hire (Hybrid is available only for Philadelphia PA)
Pay Range: $90K $105K
DESCRIPTION:
Seeking an innovative and experienced leader to join our dynamic team in Philadelphia as a Patient Care Manager. In this pivotal role you will be instrumental in enhancing the quality of patient care services delivered in a home health setting. Your expertise will guide the team in ensuring the highest standards of healthcare are met in a compassionate and patient-focused manner
Key Responsibilities:
- Lead and manage the delivery of patient care services ensuring compliance with all regulatory standards.
- Provide supervision mentorship and support to nursing staff to promote professional development and optimal patient outcomes.
- Collaborate with interdisciplinary teams to create and implement personalized patient care plans.
- Ensure precise and timely documentation of patient care and progress.
- Conduct regular audits of patient records and care plans for quality assurance.
- Serve as a communication link between patients families and healthcare providers to ensure seamless care coordination.
- Engage in continuous quality improvement initiatives and professional development activities.
Required Qualifications:
- Previous experience in home care setting with two years management or supervisory experience preferred.
- Knowledge of accepted professional standards and practice Medicare Conditions of Participation and federal state and local regulatory requirements
Required Certifications and Licensures:
- Registered nurse with current licensure to practice nursing in the practicing state.
