Job Summary
3 months contract with local authority
The role involves providing personalized care and support to individuals based on their tailored support or Reablement plans. This includes performing personal care tasks domestic duties or exercise programs designed by therapists to help individuals achieve their rehabilitation or recovery goals at home. The focus is on enabling and promoting independence. The Enabler will collaborate closely with health and social care professionals involved in the care planning and delivery of Reablement services reporting directly to the Reablement Senior Enabler/Coordinator.
Responsibilities:
- Deliver personal care and support in line with personalized Reablement plans.
- Perform personal care tasks domestic duties or exercise programs as needed.
- Support individuals in achieving their rehabilitation or recovery goals at home.
- Promote independence and enablement through care and support.
- Collaborate with health and social care professionals involved in care planning.
- Report to the Reablement Senior Enabler/Coordinator.

- Experience in providing personal care and support.

Required Skills:
Requirements: Proven experience in quality assurance and reviewing services for children and young people Strong understanding of statutory and regulatory safeguarding requirements Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment Commitment to championing the rights and well-being of children and young people
