Traditional Home Health & Hospice

Job Title: Hospice RN
Shift: Full-time part-time and per diem schedules available. Our office hours are Monday thru Friday 8:30am to 5:00pm.
Job Location: Tunkhannock PA and surrounding areas.
Our nurses see patients in the most ideal setting - where the patient resides!
At Traditional we do our best to keep each nurses case load within a sensible geographic territory.

Job responsibilities include but not limited to the following:

Admit patients to Traditional Home Health Care (Hospice) and initiate comprehensive assessments. (RNs only)
Develop and implement individualized plans of care in collaboration with physicians nursing supervisors patients and family members.
Provide direct patient care including clinical treatments personal care medication administration and patient/family education in accordance with physician orders.
Continuously assess patient condition and respond to changes by consulting with physicians and adjusting the care plan as necessary.
Maintain clear and consistent communication with physicians nursing supervisors therapists and other members of the care team.
Accurately and promptly document all patient interactions care provided medication changes case conferences and OASIS time points as applicable.
Act as a resource and advocate for patients by informing them of available services and making appropriate referrals to community resources.

Requirements:

Current license as an RNin Pennsylvania
Must have current drivers license
Ability to travel to case assignments
Excellent assessment skills documentation skills and ability to meet deadlines
Ability to work independently with minimal supervision and manage time effectively
Strong interpersonal skills.
Demonstrates good verbal and written communication and organizational skills
Acute care and/or home health experience is preferred.
Having experience with hospice patients or in the hospice environment is a plus.
Understanding of Hospice philosophy and principles of death and dying.
