JOB DEFINITION:
Integrate and provide appropriate health support services to selected caseload.
EDUCATIONAL REQUIREMENTS:
Associates Degree Bachelors Degree and/or Masters Degree in Nursing from an accredited program. Bachelors or Masters Degree preferred.
EXPERIENCE REQUIRED:
Must be currently licensed as a Registered Nurse by the New York State Department of Education with minimum of 1 year experience in the treatment of mentally ill individuals.
DUTIES AND RESPONSIBILITIES:
- Evaluation of program participants health history and needs including reactions to medication under supervision of psychiatrist or psychiatric nurse practitioner.
- Provides limited medical care to program participants as needed.
- Responds to staff requests for consultations.
- Administration of injectable medications and narcoleptics as ordered.
- Taking blood pressure and other vital signs as needed; recording findings.
- Arrangement for appropriate laboratory testing and any other medical studies indicated in collaboration with the treating physician. Implements any follow-up needed.
- Establishment of liaison between this Agency and other medical and paramedical services used by patients so as to maximize exchange of information and referrals.
- Provision of health-related information and psychoeducational services to patients.
- Orders and maintains medications and medical supplies.
- Responsible for management of medication cabinet using appropriate methods of disposal.
- Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
- Complete QI Training during onboarding and participate in QI refresher training as needed.
- Ability to cultivate a culture of inclusion for all employees that respects their individual strengths views and experiences. A Culture that makes better decisions drives innovation and delivers better business results.
- Adherence to all safety protocols and procedures to ensure a safe working environment for all employees. Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.
