Description

KeystoneCare is looking for a dedicated Physical Therapist to join our Home Care this role you will have the opportunity to provide exceptional rehabilitation services directly to patients in the comfort of their own homes.

As a crucial member of our healthcare team you will be responsible for conducting evaluations developing individualized treatment plans and implementing therapeutic interventions tailored to meet the unique needs of each patient. Your expertise will help patients regain mobility manage pain and improve their overall quality of life. Collaboration with physicians and other healthcare professionals is paramount to ensure comprehensive care and optimal outcomes for our patients.

Requirements
  • Graduate of an accredited Physical Therapy program
  • Active Pennsylvania Physical Therapy License
  • Minimum of 1 year of experience in a Home Care or outpatient setting
  • Valid Drivers License and access to reliable transportation
  • Proof of Auto Insurance
  • Strong communication skills for effective collaboration with patients families and healthcare teams
  • Ability to manage a caseload efficiently and effectively
  • Influenza (Flu) Vaccination or an approved Medical or Religious exemption required

Experience

Physical Therapy: 1 year (Required)

License/Certification:

PA Physical Therapy License (Required)
